FERPA Release Form (for current 12th Grade Students)

Seniors: As you are applying for colleges, scholarships, financial aid, etc. you will come across requests that official documentation from your high school be sent to educational institutions. The FERPA (Family Educational Rights and Privacy Act) is a federal law that protects the privacy of student education records. This FERPA form gives Southwest High School permission to send the parts of these applications (your official transcript, letters of recommendation, etc.) to the colleges or institutions that you indicate.  Please complete the gold form, including a parent/guardian signature, and return to the Counseling Office. 


You may also pick up these FERPA Consent forms from the CCC or Counseling Office.
