Athletic Forms:

A copy of an athletic physical is required in the athletic office every 3 years. Please send an email if you have questions about the date of your last physical on file in the athletic office.


 
If you participated in a sport this school year:  The Emergency Info Sheet & Participation Fee are all that are necessary for the 2nd season.
 
Participation fee's for 2012-2013 at Southwest:
* $70.00 - Basketball, Cross-Country, Gymnastics, Wrestling, Dance Team, Nordic Skiing, Volleyball, Soccer, Tennis, Swim and Diving, Performance Team, Badminton, Track and Field, Baseball, Softball, Golf
* $90.00 - Hockey
* $100.00 - Football

Bring a copy of most recent physical exam (past 3 years), and participation fee.
Each Year an updated MN State High School League packet is required available outside the athletic office or on the homepage.
No athlete will be allowed to participate in practice or events until all paperwork & fee's are completed

Clubsports of Lacrosse and Alpine Ski have separate fee's and are not-funded through Southwest High School or Minneapolis Public Schools.

Refunds are issued, prorated, and only issued if students ability to continue with the sport is not of his own choosing.
Refunds are only issued by request within 10 days of dismissal from the team.
