Auditorium Reservation Information

In order to book the Southwest High School Auditorium for any activity or event, 
you must follow these Procedures.

 

Teachers and Staff:

For use during School hours :

You can either print out and submit PDF Auditorium Request Form Staff   or you can fill out this  form online here, you will need to log-in to your mpls google account with your school log-in.  This request form asks for you to read and acknowledge the Staff Auditorium Usage Agreement (see below).

For use outside of school hours:

You will need to  print out and submit to Tory Quiggle an Auditorium Request Form with Permit.   This request form asks for you to read and acknowledge the Auditorium Usage Agreement with Permit (see below).  

Outside Groups wishing to use the Southwest High School Auditorium:

For use outside of regular school hours:

You will need to print-out the Audiorium Request form with Permit, fill out completely and either scan and email to:  Tory.Quiggle@mpls.k12.mn.us  or fax to: Tory at 612-668-3080.  This request form asks for you to read and acknowledge the Auditorium Usage Agreement with Permit (see below).

 

Below are a list of these PDF forms mentioned above.  You can print-out, fill-in and either scan and email to Tory or fax or deliver to her office (IB office, room 127).  She must receive this form in order to check availability and confirm your request.
