Forms
Many institutions prefer that students submit their applications on line.  Forms for the counselor to fill out are generally available for printing after you have completed the online application process.  If you are filling out a paper application, the counselor forms can be found in the application packet.

The Senior Counselor Recommendation and the Senior Parent/Guardian Counselor Recommendation Form can now be filled out on Naviance.  We recommend that, when possible, these forms are completed online.  If requesting a counselor letter of recommendation, both of these forms as well as the "my resume" section under "about me" in Naviance must be completed prior to the senior interview.
